I have seen this on a couple of computers with different causes. If the usb device you attach draws too much power you can get this problem. To fix it attach a usb extender that has it's own power supply. The second cause is a bad or outdated driver. Try to find the latest drivers from the manufacturer and update the driver.

Make sure that you do NOT have the wiring for USB ports connected to the wrong header on the mboard !

Some mboards, e.g. Asus models, have a firewire header and one or more headers for two USB ports that are physically identical - the pins / no pin are in the same places - typically 9 pins on a double row, 5 positions in each row, a pin missing in one corner.
If you connect wiring for two usb ports to a firewire header, or if you connect the wiring for a single firewire port to a header for two USB ports, nothing happens when nothing is plugged into the mis-wired port(s) , but when you DO plug something in, whatever you plugged in and the firewire or USB circuits will be DAMAGED in a short time !
