White box in center of XP desktop

Specs: Windows XP
When my computer boots up there is an empty white box in the center of the desktop and also a blank blue icon in the task bar.If I click on another icon on the desktop that program comes into view but when I close the program out there is the white box. The blank box in the task bar is there permanently and no matter when I click on it nothing happens to the white box the only way I can get rid of it is to right click in the task bar and click on show desktop and it goes away until I click another program and when I shut it down lo and behold there is the white box.Please help if you can it would be GREATLY appreciated.

I tried system restore twice to no avail.I can click on the other programs on the desktop and they will open but when I close them out the white box is still there and so is the blue space in the taskbar.I've tried to go into properties on desktop and there is nothing checked there.When I go into startup I do not know which things should or should not be there. Please Help if you can.

Install CCleaner, go to Tools > Startup > Windows.
Down the bottom right hand side, click on > Save to text file.
Copy & Paste the contents in your reply.
In the same window, click on > Internet Explorer.
Down the bottom right hand side, click on > Save to text file.
