Have a suecure password try to include lower case UPPERCASE numbers( 123453123) and symbols &!@ example P4s5w@rd


make sure your security questions are god and never give out the answer by mistake, you could always apply what i said about to the security questions e.g. Frave food? P1zz4.
Same with facebook security question.

More important to protect the pc you are on that accesses these accounts. This is why people install software firewalls.

It's possible your system is presently infected so any password changes you make won't do any good. Have you scanned your system with trusted anti-malware programs like Malwarebytes and SuperAntispyware? The latter will also scan for tracking cookies, which aren't technically malicious, but it's good to clean those up regularly, too.

Many of the sites don't offer encryption to the server. It may be possible that your system has a key logger or other means to intercept the new keys.

It is possible that you have a keylogger in your system.. you may got it from downloading pictures or videos from the internet. Try to reformat your pc
