If you've had it for a while it may have reached the end of it's life. There is a limit to how many times you can write to it, and a limit to how long it will retain the data that's on it.

I have found the original low capacity ones to be far more reliable, I'm still using a 1GB one after 5 years daily use, whereas the 8GB one I bought didn't last more than a year. So I no longer buy them. I use external hard drives instead. They cost more initially but are more reliable and last longer so you actually save money over the longer term.
