Windows Batch- Convert Row To Column In CSV File

Specs: Windows 7, 4
I have a an excel file which looks something like this:
A B C Measure Names Measure Values
Business Associate AB Inquiries 30,356
Business Associate ZY Master Lead 5,387
Business Associate XY Shipped Leads 6,430

Here I have written a batch script to achieve the above result in csv file. The final part is I want column 4 and 5 to be pivoted.
So the final output will look like this:

A B C Inquiries Master Lead Shipped Leads
Business Associate AB 0 0 1
Business Associate ZY 0 0 10
Business Associate Xy 0 0 14

Can some one please help me with this. I am not sure how you will do this using batch. Again I need to pivot only the column 4 and 5.

Here in the 1st table column 4 and 5 are : Measure Names and Measure Values from the 1st table.

I'm not sure asking the same question three times over two days is going to encourage anyone to help you, but I've been wrong before. You say batch script, but you're also insistent on Excel, so maybe the Office guys have an answer for you.
