How do I make this webpage? I'm lost.

I received an assignment in programming class where I have to make a web-page that calculates payroll for someone and I don't know where to start. here is the layout:

I need an input box for Hourly Wage and Hours Worked. When those 2 boxes are populated, it performs tax deductions and shows those results along with net pay and gross pay. then There needs to be a submit button at the bottom that exports the results to a text document. I have never done this before and I don't know where to start. Please help if you can.

If this is to have a Web page interface, that will be HTML with a <form>, several fields, and a button or two. The HTML page can be hand coded, use a page editor, or use a scripting language such as PHP or Java Server Pages to build the HTML. The form's "action" will send the data fields to a form processing (CGI) script on the server. This could be PHP or Java, among other languages. It will have the interfaces and processing ability to do the number crunching, and can write out to a text file.

The number crunching can be done on the HTML page in Javascript (this is not the same as Java!), but cannot write to files on the PC. As this is a Java class (?), the teacher's intent might be to have you write a Java applet that would be invoked by a web page. You might want to ask for clarification. Is this a Java class, a web page creation class, or something else?

It's an introduction to programming class. He said it would be easiest to do calculations and stuff in Java, but he wants it hard-coded so there is only 1 file.
