I know this may seem like a silly question, but I have had it happen before, is your printer a color printer or does it only print in black?

Also, make sure that your image within the document isn't set to grayscale when printing.

I'm guessing it's a printer driver issue.

We had problems with our company logo and other graphics printing as black boxes or as separate elements with boxes around them. It was happening with PDF documents as well as PowerPoint documents.

It turned out to be the generic print drivers our IT department tried to use to eliminate different drivers for the various models of laser printers we had on our network.

Once we standardized on a single printer model and the proper driver, the problems went away. I'm guessing we're good for a few years until a printer breaks beyond repair and that model is no longer available. After that, we'll have a mix of printers and drivers and problems will surface again.
