Conditional Formatting (Icon) on a PivotTable

Specs: Windows 10
Hi, I'm trying to use an icon style for conditional formatting, but I'm having hard time doing it. Here are my conditions:

If the value is >=98% (Green arrow)
If the value is <98% and >=95% (Yellow arrow)
If the value is <95% (Red arrow)

Also,why are the icon styles disappearing when I refresh my PivotTable. Is there anyway to avoid this?

Applying Conditional Formatting in a Pivot is not as straight forward as doing regular CF
