The only way to unlock an iPhone properly is to have the carrier do it if they offer that service, some do, some don't. If you paid for it, it was done by someone who just hacked it, you got taken. Hacks rarely work properly (as you've just found out) and go back to being locked if you update the operating system.

Who is the carrier on the phone?

As far as I know, AT&T does Not charge to unlock, as long as you've been with them for two years or more, they unlock for free. And if it is officially unlocked by AT&T, you don't need to jailbreak.

"I contacted iTunes and they also said it was successful".
iTunes is a program used to download music and transfer it to your iDevice. It is not a company you contact to verify unlocking. If you meant you contacted Apple and they verified it, then you are definitely lying, Apple has nothing to do with unlocking* or verifying it being unlocked.

* You can buy unlocked phones from Apple but they have nothing to do with phones that have been purchased from a carrier that are already locked.

Not to be rude, but nothing you have said so far makes any sense at all. You've either had someone hack your phone or you have jailbroken it. Either way, it sounds like its hosed.

You can try a factory restore, it may set it back to normal but it will be locked to AT&T.

You've already been helped as far as is possible with your problem, and THX 1138 has suggested you try factory reset/restore. Seems like a "good idea" to me since you've nothing to lose.
