Specs: Windows 10, 16gb ddr4 at 2133mhz
I have a 16gb ddr4 at 2133mhz ram and a nivida gtx 1070 and Intel I-7. I play games like rainbow six seige and csgo (first person shooters). I get around 40-60 frames on high settings. My game is ok and suddenly will stutter for a few seconds. What should I get so I have flawless gameplay. Also have a 144hz minister with vsync. And should I use v sync and in what situation.

You have the hardware you need. Make sure you have it configured correctly for a 144Hz monitor & that you're using the proper type of cable - either displayport or HDMI 1.3 or better. Also make sure you have the BIOS settings configured correctly, don't simply load defaults & think everything's good. Defaults are just a starting point. The settings should be configured to your hardware.

You should also do regular system maintenance. Uninstall unused programs, disable unnecessary startup entries, run a cleaner such as CCleaner-Slim, install an AV program that's light on system resources (not Norton/Symantec or McAfee), make sure drivers are up to date, scan for malware, etc, etc.
