IBM Thinkpad fan noise

Specs: Windows XP, Intel Core 2 duo 2.0GHz/1GB Ram
My brothers IBM Thinkpad T60p fan randomly starts making odd noises...a pulsing whining kinda noise. We have cleaned and remounted the fan, twice.
This doesnt happen everytime he plays (he uses it for gaming)...maybe once every couple days.

Any ideas?

EDIT: It started making noise again today, he rebooted the system and then got a Fan Error and now wont boot at all.

I have a T60 87445BU model. I have also had the rattling issue with my fan exactly how you describe. What level of cleaning have you done to the fan? I have taken the fan assembly out, then bending the 4 small copper tabs the fan itself opens like a clam. I then take the fan off, only held on by a little magnetic force. I then used a q-tip to clean the dirt out of the fan (around the shaft), then I applied a very tiny amount of lubricant. I have remounted the fan and it has solved the problem. It seems like most people on forums are replacing the fan for $90+ but this took me 15 minutes and works great.
