Wireless card detected, then not

I have an IBM ThinkPad T41 with a Cisco Aironet 350 Mini-PCI card. Sometimes the card works perfectly and other times I boot Windows and the "Found New Hardware" wizard shows up. If I cancel the wizard and reboot, it may or may not start working again. In other words, every time I turn on the laptop, there's a 50/50 chance that the wireless card will work. What could cause this? The drivers are up-to-date and this is a fresh install of Windows XP SP3 with all of the Windows updates.

Faulty wireless card. If is under warranty have IBM fix it, if not then get one of the notebook cards listed at the site below.
