To get the maximum performance out of your computer, initial and maximum values should be set to the same thing; dynamically expanding the virtual memory space eats up performance like nobody's business. Generally, the "recommended" size (1.5x your amount of physical RAM) is perfectly fine. Remember to click "Set" before hitting "OK".

The best pagefile setup is a system managed one.

I don't suffer from insanity, I embrace it.

There should be an "In my opinion" in there somewhere.

Well, as I said before, having the system manage the paging file is a Bad Thing if you're going for performance. You might experience lockups or slowness if Windows decides to expand the paging file while you're doing something system-intensive. This also results in the page file becoming fragmented, which slows down performance even more because it's not all in one piece on the drive. Generally, the few possible problems that can come from a fixed paging file far outweigh the woes you'll experience with a system-managed one.
