Comment:

In TweakUI (part of Powertoys for Windows XP), I used the "Rebuild Icons" feature (under Repair tab). Afterwards, all my icons in Windows Explorer (on desktop, taskbar, other places with file icons, etc.) are in 4-bit (and they look really crappy). My display is normally set to 16-bit. If I switch to another color depth, the icons look normal in that color depth; but if I switch back to 16-bit, the 4-bit icons are back. Any help would be appreciated.

This answer may help you out but you may have to experiment with it for awhile.

Open Start Menu, Settings, Control Panel, then click the Display icon.
Select the Appearance tab.
Select the advanced button
Select the icon in the Item Box.
Increase the icon unit by 1.
Select Apply.
Decrease the icon unit by 1.
Select OK.

Didn't work; changing the icon size only stretches or unstretches the messed-up icons.
