Perhaps it is not available in XP Home but my XP Pro has 'Narrator' which is in Start\All Programs\Accessories\Accessibility. It says it "Reads on-screen text, dialog boxes, menus, and buttons aloud if speakers or a sound output device is installed."
Is that what you are looking for?
If it is not there perhaps you can go to Add\Remove Programs and the Windows Componets section and install it from there.

I just purchased a new office suite called EASY OFFICE which comes complete with speech and voice command. It also comes with more than what you get in an average suite. You can get the limited version as freeware, but for full voice cammand you need to purchase ($45.00 US). It's worth it though.
