Refresh rate/FPS problem

My monitor's current refresh rate is 75 hz, but the maximum FPS I can get in games is 60.
Why is this?
I admit, I have a pretty bad video card (64 MB integrated video) but it should be able to run Half-Life, of all games, better than 60 FPS, considering my old computer (933 mhz with a 16 MB Rage 128 Pro) could get 60-99 FPS (depending on what was onscreen).
