MS Word form alignment

To all:

Using MS Office 2003. MS Word is realigning and resizing a "state generated", "fill in the blanks" form. There are no formatting options available on form, only filling in data. When Word opens file, the image actually shifts toward the left margin, leaving is 2+ inches on the right side of page. Page also samller than 100% size. Have looked into print options and Word setup options without success. Any help appreciated.
