Why is it that when you convert any vob file to another format there is always a place in the movie that is messed up or skips?

How would you cut out parts of a movie you didn't want? like previews or disc menus?

Let's say I wanted to put a dvd onto my harddrive, it contains 4-6 episodes, How can I extract just the episode and not the disc menus or previews, also parts of an episode is broken down into 2 parts. example episode 106 ends and 107 starts then stops, then starts playing from another vob file. basicly what im trying to do is extract all of the episodes off of a dvd into a single file for each episode format like avi or mpeg.

plenty of dvd ripping/re-mastering tools out there. best site for dvd related stuff would be doom9 ... tons of downloads, guides, forums, etc.
