Use the search function at the top of this page and search for boot.ini. This question has been asked and answered many times. Lots of help there.

Boot with the Windows XP CD, and let it load all the initial files. You will then see a screen that asks if you want to Repair Windows, press R, or if you want to continue on to Setup, press another key. Press R key for recovery console.
Once in recovery console, enter buildcfg /rebuild and follow the prompts to add your windows installation to the boot.ini file.

The buildcfg /rebuild will automatically create the boot.ini file for you.
You could manually create the file using notepad. This configuration is for 1st hard disk with the first partition on it having windows installed in the \windows folder. Insert the following lines and save as boot.ini :
[boot loader] timeout=30 default=multi(0)disk(0)rdisk(0)partition(1)\WINDOWS [operating systems] multi(0)disk(0)rdisk(0)partition(1)\WINDOWS="Microsoft Windows XP Professional" /fastdetect
You can then copy the file(boot.ini) from floppy disk to the root folder of your hard drive c:\ using bootdisk if your original partition was formatted using ntfs.
