Blank error box on start up

OS: Windows XP home
CPU/Ram: 1.8ghz 256 mb ram

When I try logging on to my Win XP, it starts up, but then I get a blank dialog prompt that has two choices, Yes or No. For a while I could click Yes or No and it would logon. But now, if I click either of them, it logs me off instantly. I've already tried using the System Restore, and that didn't work. I can only logon in Safe Mode without networking. I have re-formatted my computer 5 times now and I get about two weeks grace before the blank box appears again. Any help would greatly be appreciated.

Assume you have checked for viruses, adware, spyware, etc. If not, post back.

Salgolf, yes I have Norton Anti-Virus 2004 constantly running in the background, I also run Ad-Aware Professional and windows washer daily.

I have exactly the same problem, but it seems I am on the "early stage" and I can still log on. Any help please?
