Unable to change default programs in Vista

Specs: Windows Vista
I'm finding that when right clicking and choosing "open with" doesn't work for any thing. No matter the file type, and no matter which program I chose to open it with. Once I browse to the program and hit open it simply goes back to the "open with" screen and the program I picked doesn't appear. I always have to open the program I want, then find the file through the start button and manually drag it over to the program I want to open it with. That's about as clear as I know how to word this question, hope it makes sense. Any thoughts on a fix? I can't get it to work by going to "set your default programs" or "set program access" either. It seems like Vista only wants you to use the programs that it has set for defaults. Any I'm pretty sure that I use to be able to change the default programs, I'm not sure when it stopped working. shinjots, What file extensions are involved in this problem (.exe, .jpg, .txt, etc)? The link below has file association fixes for many file extensions along with Usage Instructions. They would correct the problem.
