I keep losing audio & video

I have been running Vista (Ultimate) for about a year now. Ever since I installed Vista I have been having the same problem. Usually, (not always,) audio and video works fine upon booting up, but only for a few hours or occasionally, for an entire day. Then suddenly, the audio won't work at all and video runs at about one frame/3 sec., if at all. MOST of the time it will work again after rebooting, but not always.

One thing I have also noticed... if I am viewing video and pause it, it will usually stop working at that point.

I've even noticed that when playing solitaire sometimes the sound effects will stop working in the middle of the game. When it does I will often check and find that my video has also stopped working at that point.
I couldn't tell you how many time I have uninstalled and reinstalled drivers, this seems to not be a long-term fix. I would really like to find a solution to this once and for all.
