I am getting a message that says "Access violation at address 13161F1D in module 'PI5d2_284.exe'. Read of address 00000000." What does this mean?

That doesn't look like a normal Windows file. Scan the system with your AV, then download, install, update and run MalwareBytes freebie. The latter is good at finding and fixing things that your AV can miss.

Err...I'm assuming you can boot to Windows. If not tap F8 key and select Safe Mode from the menu. Then do the above from there.
