I have a dual (C:\Windows NT 4, SP 6 & D:\Windows 95). I recently formatted C: drive and restore Windows NT on it. The problem is that now Windows NT can’t access D: drive. Any help would be appreciated.

But you can still boot either? Are you using NT boot loader to dual boot - or third party boot manager (or just swap boot drive in the bios)?
what do you mean, can't access?
presumably win95 install on fat(16) not fat32 filestore? (eg, you didn't previously have something like www.sysinternals.com utility which allows NT to read fat32 filestore?
