Whenever I want to save an picture in MS Paint (no matter what format) using the filename blabla.04 it will save the file as an .04 file (and not the correct picture format). Saving the picture with another filename like blabla.05 or blabla.06 will save the file with the CORRECT format (bmp, jpg, etc).
I have checked for file associations, reinstalled paint, but no luck.

It sounds to me like your Windows NT has some program installed that associates files with a .04 extension. It is far easier to fix this with the old File Manager (carried over from Windows NT 3.x) than it is through Windows Explorer. Simply do the following:
From a Command Prompt window, type winfile. This will bring up File Manager. Click File - Associate, and click the dropdown arrow next to "Files with Extension". See if 04 is listed; if not, type in 04. Under "Associate With", scroll up to (None) at the top and select that. Click OK and this should fix the problem.
