tape drive error detected

OS: windows NT 4.0
CPU/Ram: 180 mhz, 64mb edo ram
Comment:

When trying to do a backup of the server I get the following messages:

Tape device reported an error on a request to get device status.

Error reported: hardware failure

Event ID: 11
Source: 4mmdat

Desc.: The driver detected a controller error on \device\tape0

Also, before I got the following message:

Event ID: 7026
Source: Service Control Manager
Desc: The following boot - start or system - start driver(s) failed to load: 4mmdat

Any help would be appreciated.

It is pretty clear that the OS can't talk to the tape unit. Is the tape unit on and have proper termination? Cable connecting the scsi device OK?

Do you see, when you boot up, your scsi cards announcement and does it list the tape drive?

If it does not list the tape drive you either have a tape drive failure [replace it] or cable/terminator have failed.
