Dr Watson Access Violation

Whenever I right click on the task bar then click remove on the Start Menu Programs tab, I get the following Dr Watson error,
Explorer.exe
Exception: Access Violation (0xc0000005),
Address: 0x715a5f94

I get dumped back to the desktop and half of the system tray icons dissapear.
Not sure when this started happening as I don't edit the Start Menu often. Tried uninstalling the last few programs but didn't help. It's not the profile cos it happens under local Admin, Domain admin and under my User profile too.

Searched Microsoft but didn't find anything that quite matched or helped. Anyone here know what's up?

Using NT Workstation 4, SP4.

The MCSE in me says first to upgrade to SP6... I would also try to run a chkdsk to make sure the HD is ok.

Just tried the chkdsk, no help. Tried upgrading to SP5 yesterday, didn't cure it either.
Can't get SP6 at the minute, our company internet connection is down. Will get it over the weekend at home and try on Monday.

Yes, perhaps a bad profile, which can cause lots of trouble, but make sure that you are in the local admin group as well

can I do?
've also received "Windows NT error code
= 87"

hi , everyone
i am being faced with a similar kind of problem , we have a setup of around 360 workstations(winnt4), offfice 2000+sp6+all latest office service packs ,we have tried even a different copy of office 2000. still the application error (access violation does exist)persists.From the drwtsn log filewinword.exe appears to be the offending application, in some cases explorer.exe, systems just hangs up if i open more than4 word documents,can
anyone tell me how to analyse the dr watson log file or any other suggestions
microsoft site has nothing to say other than
service packs

iam trying to use an application monitoring tool today shall post the results after 1 week

I am new to list and have one question about the Dr. watson crash and the Drwtsn32.log produced after thr crashed.
I just want to understand the contents of the "Drwtsn32.log" file.
The Drwtsn32.log shows all the .exe under "*----> Task List Task List
