I am not 100% sure but when we had an Exchange Server and wanted to mail outside, we had to make an entry at our Novell Server, mapping the inside network address of our Exchange Server to a valid outside IP address so that when mail came in, address translation at our proxy server redirected the mail to proper inside IP address. You might want to check your proxy server settings.

If you are using DNS then you need to create an MX record pointing to your Exchange server for incoming mail and an outgoing Host address to send your mail on its way.
