I strongly suspect that your Word document template is corrupted. It seems to be rather common, as I was having trouble with Word not opening myself, a few months back. Correcting the problem is so simple you won't believe it.
Go to this site page and then scroll down to #2. Under #2, read item i) (the page is presented in an outline format).
