You seemed to have tried everything. Have you run the test found in DXDIAG? if not, type that into run on the start menu,
choose "DISPLAY" and run the 3D tests, also check all the DIRECTX sub-menus for errors.
All I can say is that eventually, you'll figure it out. (If you live that long:)
I had the same problem when I updated my video driver, it seems it wasn't compatible with my version of DIRECTX, I had to
uninstall the newer version of DIRECTX and install an older one, go figure!
