hy i've a pc shop and i've a problem.
Some times when i build a pc , at last of the process (at reboot of machine) , a mistake comes visualized on screen : during ios device inizialization : Mistake , a driver of i/0 subsystem has not charged correctly . a file inthe directory \iosubsys can be damage or the memory is finished.

my question is : but this problem was not only with win95 and amd k62-350 ??
and i have win98 ! then why ?

my useful configuration is cpu pentium2 350 , m/b intel se440bx2 kingston's ram , matrox millenium g200 or another and cd-rom asus.

can someone help me ?
can i find a patch for win98 ?

I get the same error with AMD K6 400 I just built
tried switching hard drives, still get the same error
switched video cards, still same error
even switched the hard drive to another system , different chip
I would then assume its a hard drive failure, like a boot sector

I have had this problem 3 times on my PC now and the only way I have got round it is by reinstalling.

If anyone actualy knows how to fix it less drasticaly it would save me a lot of grief !!

I am not able to start my cd rom because I
must have lost a file in thje ios subsystem
The file I lost is mtmminip.mpd and without
this file I cannot even load a disc into the
D: drive. It does not recogzize the drive. It
says the D: drive is not accessible. How can
I get that file to add back into the system?
If not, I will have to completely reformat the whole hard drive and lose all material and such.

I too, have got the IOS error. At bootup on a AMD K6-II 400. Changed the SMARTDRV.exe and still run into problems with it with Win 95 B.

I installed netscape communicator and there it was...IOS error... I trimmed my computer a bit and I got it working, except without sound. I didn't change any files or anything, just "turned the soundcard off"... sorry 'bout my language problems because I'm 17-years old boy from finland and I can't speak english very well. But anyway, how can I get my soundboard "alive"... nothing has helped me from these answers above....

It's me again... I've done it... the error has been resolved... just write in your system.ini(right after the vcache thing) that maxfilesize=6144 and before that turn your soundcard out of use. this should work... at least it worked for me... tried to figure this out for about 9 hours... but now I hope that it's useful for others ... have fun with your i/o errors... =)

same IOS problem here too.
i have win95 + asus p5a-b equipped with amd k6-2 400 mhz. configuring system.ini or smartdrive.exe didn't work for me. i've tried everything to fix the IOS-problem and nothing worked but the following:
1. set the cpu speed to 300mhz on mboard
2. install win95 (again i suppose...)
3. download win95 amdk6 update from internet and install it (microsoft support-page didn't work!!!)
4. turn cpu back to 350 or 4** mhz (whatever)
5. turn your computer on and say hello to
allmighty WINDOWS95 ...i hope so...

My configuration is AMDK6-2-333(4.5x75), 64MB SDRAM, 2.5GB HD.
My problems started when I upgraded Win95OSR1 to Win98SE. Now I got that above mentioned IOS error every time I boot. The spice in that is that booting goes without errors when I boot machine by holding CTRL-key. My system is very stable, there's only this booting problem.
I have tried so far these steps:
- renamed SMARTDRV.exe
- Renamed RMM.PDR (in \iosubsys)
- Checked system-files using win-utility
- Done registry-checking
- Set VCACHE-parameters (maxfilesize)
- Loaded Win98 system-update + Y2kfixes+ IE5.0

All these efforts were done without success.
It seems that in SYSTEM my power-saving features are disables even though drivers are loaded for that. In BIOS I've disable all these features and I don't need them. I'm wondering if this might have something to do with the problem. Haven't done anything yet to prove that.

Without fail, I have the same problem. I have an AKD k-6 2 400mhz clocked down to a 366 cause of motherboard limitations. I have tried win95 a&b, win 98 a&b. all with the same trouble...error loading device iosubsys; you must reboot and start over. (all the time when loading windows)!

I don't know if my resolve will be a fix for every body, and surely I've tried the trick mentioned: renaming smartdrv.exe, setting the maxfilesize to 6144 in the system.ini, and even deleting rmm.pdr in the windows\system\iosubsys directory.

It was perhaps by accident that I found this, but if I boot up, press f8 and boot to a command prompt, I would then type "win" to enter windows, and no iosubsys error whatsoever. So now I added to the last line of my autoexec.bat file the line:

WIN

this automatically starts the loading of 95. Even though win starts by itself, this is the equivalent of starting windows from the command line which seemed to curve the iosubsys error for me. I have no problems whatsoever with any of my hardware before or now.

I have an AMD k6-2, 400 mhz processor under the hood. I have 96 mb of ram, 16mb video card, sb pci128 sound card, Hauppauge wintv card, a 32x/6x dvd-rom drive, an external hp 7200e 6x2x2 burner, and an info scanner attatched to my system. finally, I have a network card that is attatched to my intel pentium 90 so that I can run my Epson Stylus Photo 700 printer (I ran out of interrupts on my Amd machine, and this is the easiest way to have everything installed heheheee!)

Pleasereply to this and let me know if you have any success with just starting windows from a command prompt or adding "win" (without the quotes) as the last line of your autoexec.bat file.

Try this:
This problem can be worked around by adjusting the maximum cache size allocated
by Vcache.vxd. Vcache.vxd allocates an initial cache size that is no larger than
9MB. Setting the maximum cache size to less than 9MB also reduces the initial
cache size, freeing up memory below 16MB and allowing Windows 95 to boot
properly.

The maximum cache size is set by adding a "maxfilecache" line to the vcache
section of System.ini. The following example shows what an entry in System.ini
would look like to set the maximum cache size to 6MB:

maxfilecache=6144

If you are reading this, you should be aware that two very different problems are being discussed here. You probably DO NOT have both of these problems on any one machine. I have experienced both of these problems (on diffenent machines) and know that their error messages are simular. My experience was with 2 very different machines.

One problem deals with the AMD processor running at 300Mhz or faster and is a TIMING issue. Toby's response is correct microsoft has a download to fix this and it has nothing to do with smartdrive.exe or maxfilecache. If you have an AMD running at 300Mhz or faster this error may show up only every once in a while.

The other problem deals with a machine that probably has less than 64MB of memory, may or may not be an AMD and is probably running Win 95 OSR2 (B). This problem shows up every time you boot, with out fail. This problem is corrected by renaming the smartdrive.exe file and setting the maxfilecache as discussed above.

I am trying to fix my friend's Toshiba P-100 Satellite Pro 420 CDT laptop (only 8-meg ram) that began having this problem last week right after using AOL 5.0. Until then all was well. So far non of these solution work.
Do I tell her to shell out money for more memory, and promise her that will solve the problem?

I have Pentium-II Intel SE440BX2 having 64MB Ram, 6.4GB HD, Zoltrix TV Tunner,US Robotics 56K Modem,Trio 3d AGP, PhoneJack and Win98. Some Days ago bios erased which i recovered. After that I refresh my HDisk and made partitioned it again installed all softwares and drivers many time but the fatal errors do not leave me. Some are given below

OK - I had the same problem, but couldn't even boot into safe mode, only to DOS. I did vcache, rmm, smartdrive, etc. I also did a registry restore and a scandisk.I listed the files in windows/system/iosubsys with their dates and sizes and found that they matched exactly those in another W98SE PC's subdirectory. So I took to heart the part of the message that says that a file in the subdirectory may be corrupt and (after copying the originals to a diskette) transferred mine to the non-booting PC and the problem is gone. Here's a list of the files.

As everyone else, I am having the same problem. However, I'm getting the infamous message even when going for safe mode. My ? is: how can I bypass this message if I can't even boot up ANY form of Windows? I would have absolutely no problem formating and reinstalling, exept that I do not posses any form of WIN95 that isn't on my recovery CD. As you are all aware, my CD is not even loading. Any help would be accepted. I'm working on a 60Mhz 1.2Gig system.
