I was getting this error when clicking on any .wav files after a clean install of Win98. I thought you'd like to know the solution.
I upgraded my PCI sound card drivers and found this cured it. My card is a Trident 4D-Wave. Trident have just released drivers with Win98 updates.
Get the drivers from
http://www.tridentmicro.com/default.htm

I was having a similar problem but rarly got the fatal error. She would just lock up tight when certain(not all) wave files were played. The new driver seems to have worked.

Solution at long last!! I recently upgraded from 32mb Ram -> 96mb. Unfortunately the I purchased some slightly different speed Ram - The only way I could get the motherboard to recognise it was to set SDram Latency to 3 in my bios - I've had no crashses at all this week.

Okay...I have had this problem for awhile now and finally figured it out. First off my system

P3 450
Tyan 1854 MB
128 RAM
V770 Ultra

The Solution for me was to down load the AGP drivers for the VIA chip system on the mother board. Fixed everything. Hopes this helps

Socket 7 Motherboard AGP Video Card Installation Guide:

1. Windows95 OSR2.1, Windows98, NT4 w/ Service Pack 3 (or SP3 +SP4)

2. All motherboard patches installed:

VIA MVP3 chipset-http://www.via.com.tw/drivers/index.htm

VIA AGP driver
VIA IRQ Routing driver
ALI Alladin V chipset--http://www.ali.com.tw/eframes.htm

ALI IDE driver
ALI AGP driver
ALI USB Host Controller Supplement
PC100 AGP Pro --http://www.amptron.com/html/drivers.html

USB Support Drivers
AGP Patch file for AGP MBs
PCI IDE drivers for Pentium
Soyo ETEQ chipset -- ftp://ftp.soyo.com.tw/drivers/eteq/

IRQ Redirection patch
South Bridge Identify patch
IDE Bus Master/Ultra DMA Drivers
Eteqvxd.exe AGP patch
There are also some motherboard bios options that you need to change to keep the AGP socket running at 66mhz. Please keep in mind that all AGP 1x,2x,4x AGP cards run a 66mhz bus speed, it is the texture transfer rate that the 1x,2x,4x refers to. The first thing to look for an option of AGP 2x or AGP Turbo and DISABLE it. After that, if you have a VIA MVP3 chipset, reinstall the AGP driver in NORMAL mode. If you have a ALI chipset motherboard, you also need to DISABLE the following bios options: Primary Frame Buffer, VGA Frame Buffer.

Lastly in the bios, make sure the option "assign IRQ to VGA" is enabled for the Viper cards. For the Stealth or Monster Fusion cards you can leave this option on Disabled.

3. Microsoft(r) Windows(r) 95 Update for AMD-K6(r)-2/350 and Above--http://www.amd.com/products/cpg/k623d/win95_update_k6.html

4. Latest motherboard bios update

5. Latest Video card driver--http://www.diamondmm.com/products/drivers/driver-index.html

For AGP to work with Windows95, here is some basics for any AGP card (socket7 and PentiumII):

1. Windows 95 version OSR2 (950b or 950c)

2. USB update to upgrade to OSR2.1 (on the windows95 cdrom that says "With USB support" on the front in the X:\Updates\Other\USB\USBSUPP.EXE or from this Diamond site--ftp://ftp.diamondmm.de/hotline/files/usbsup.exe) To check for the OSR2.1, START>RUN>"qfecheck". The windows version should be 4.10.01.1212b or 4.10.01.1212c

3. Chipset patch for the motherboard installed-ie LX, BX, VIA MVP3, ALI V (if not loaded, in Device Manager will show up under Other Devices as PCI BRIDGE)
