Device IOS error on startup of win95

I get a message:

While initializing device IOS:
Error: An I/O subsystem driver failed to load.
Either a file in the .\iosubsys subdirectory is corrupt, or the system is low on memory.

I can boot to safe mode but not normal. Any fixes? I recently installed IE 5 and all updated accessories.

Howdy, Well I have you updated any drivers latley? What is the last thing ya did before this happened? I doubt IE 5 has anything to do with it. Can you look at you're bootlog.txt file and see what is not loading. There are lots of protected mode drivers that are loading so you are going to have to do more research before anyone can give ya a responce, that or take it to a tech. If you can give more details it would help.

Protected mode drivers have the .pdr extension and are in system\iosubsys. look at the text files IOS.*

We have seen many IOS errors most are caused by corrupt netcard drivers, but can also be caused by bad ram. See the link above for all of the IOS errors we have seen.

I am receiving the same error message as Kevin Smith and can also only boot in safe-mode. This problem only occurred after I upgraded to IE 5.0 also. I think there may be a trend starting.
Any advice??

If you have an AMD K-6, it can cause that error too. You might wanna try AMD's pathch for kicks > andk6upd.exe - it's available at both amd and microsoft sites.

computer boots in safe mode ,but would notlet me in..SO ,did a format loaded dos 6.22 &win 95.reboot get the safe mode again.
won't let me in to safe mode and says

The folks at my company's help desk had me edit windows\system.ini. Under the [vcache] section, I added the line (there was nothing there when I started):
maxfilecache=6144
Worked on my Compaq Armada.

I get this problem too. ONLY after installing IE5. Can boot in safe mode only.
If I un-install IE5, still get IOS error.
I have AMD K6 2 3D 350MHZ chip. AMD patch NO difference.

The only thing I can do is, re-format my hard drive. I've done this about 3 times now.

I have Windows 95 installed B version.

HELP.

go there...this is a problem with OEM versions B and above of win95...not win98....if you have version A or the upgrade to win95...well i dont know...get a new version.

Well, I must tell you that IOS error can happend on any Intel 66-133 non MMX CPU and AMD K6-older once. On any other system CPU will not couse this error. What can couse this error can be anything, even bad RAM or video card on the board with 1mb of video RAM. My sugestion is to take out of unit everything expet video card. If that does not help remove half of RAM( Very common error if you mix brands of EDO RAM) Packard Bell Legend 2450 and higher runing win95 B or win98 after installation will boot with this error but after restarting in Safe Mode should fix this problem. Windows 95 first version usualy won't show this error message. IE 5 can activate this error message but won't couse it. I would sugest installing IE 5.0 only after you reinstall or install OS . If you get a message DO NOT RESTART IN SAFE MODE< ALWAYS CHOSE NORMAL MODE> DO IT FEW TIMES AND WITH NO EXPLANATION MESSAGE WILL STOP COMING.

I dont have any version of IE on my computer and I'm running W95 on an AMD K6-2 400 chip. Ever so often off a wamr boot I'll get and IOS ERROR on startup and I have to reboot, then it doesn't happen again for a few boot ups. Is it possibly the chip causing this error? I know that the ram is good, because i took it from my previous computer and it worked perfectly. if it is the AMD chip, will their patch work for it?

I had the same problem when installing VS Installer.
Under the [vcache] section, I added the line
maxfilecache=6144. It solved the problem.

After installing Internet Explorer 5 with Outlook Express 5 upgrade, I got:
Error: An I/O subsystem driver
failed to load.

I have over one Gig of hard drive space available so I know it isn't that.
I am not happy.
What is the solution to this problem?
I find some humor that the CD-ROM also said it features Y2K compliance.

maxfilecache=6144 solved the problem.

I thought the 6144 solution wouldn't work but it did. Thanks! I think the problem started when I instaled RealProducer 7.0 Beta but I'm not sure. Hope this can help someone else.

Hi all,
I received the same problem on Compaq Deskpro EP Series computer.
The solution was, as mentioned by others, to edit windows\system.ini file and add the line:
maxfilecache=6144 under the [vcache] section.

I got an SB PCI128 card a couple weeks ago and had no problems after loading the drivers, but when I installed Visual Basic 5.0 last night I got the dreaded IOS error. Removing the SB drivers makes the problem go away so it's connected to (or triggered by) them. I'll try the 6144 solution to see if it works, and if not then I'll try another proposed solution for errant SB drivers: Editing EAPCI.VXD with a hex editor and changing "nuwave.drv" (which does not exist) to all zero's.

I had the I/O Subsystem error numerous times on my company's Armada 1500 class laptops. All run Win95 OSR2. It happens on IE5 and Visio 2000 installs to name a two occurrences. I've seen some installs actually remove the fix listed below.

Fix has always been to add the following to system.ini:

[vcache] maxfilecache=6144

While Initializing Device ISO: Windows Protection error. You need ot restart your computer. I cant load windows in safe mode to install amdk6 update patch

Some of the problems I've just read I'm sure are related to a problem w/win95. The amd patch is recommended for systems at 350 mhz or faster. Upgrading to 98 can also help.

OK same thing happened to me on my Armada.
I did the "[vcache] maxfilecache=6144" thing and it worked!

It's now about 8 months later, tried to install a music program and the same thing happened again. "[vcache] maxfilecache=6144" is still in my system.ini file (followed by minfilecache=6144).

Checked bootlog.txt - loads of fonts wouldn't load (failure code 0016) - is this coz I is in safe mode??
Checked bootlog.prv - setver.exe failed, ndis2sup.vxd failed and vshare failed.
I renamed smardrv.exe - no change.
I renamed rmm.prd - messed up my file names (turned them into 8-bit format) and no programs would work (kernel32 error...).
In DOS renamed rmm.prd back to what it was, restarted in safe mode and swept for viruses. Didn't find any.

Removed the "minfilecache=6144" line from my system.ini files and used "Tweak UI" -> repair tab -> repair system files.

SORTED!!!

Thanks for all da ideas...love ya...
(u can find all the above files using the find command on the start menu and u can use yahoo to find Tweak UI)

Having tried all the options on the said problem and failed! I then tried to install windows 98 direct from the CD and 'BINGO' it loaded OK, and the fault has been removed. So if you have win 95 and Win 98 load them straight after each other and you will not get the same problem.

Wow! I work in Internet tech support and I just took a call from a customer who was getting the iosubsys error and could only start in safe mode after installing our cd (good lord!).
I had heard of this error before and knew of no solution. I happened to find this page as I searched for information on it on the net and the system.ini vcache edit worked liked magic!
For those who don't want to scroll up to find it, aloow me to quote from an earlier post:
Under the [vcache] section, I added the line (there was nothing there when I started):
maxfilecache=6144

Had the same problem after installing Office 97 Pro on Win 95 OSR 2.0.

The [vcache] patch solved it for now.

I tried the vcache fix on a generic Intel 400mhz system. Worked fine. I LOVE THIS SITE!

One thing I noticed is that rmm.prd was updated at exactly the same time as I ran setup for an older version of MS Office. I'm guessing that rmm.prd can get replaced/corrupted by a variety of setup programs, from IE5 to MSOffice and probably others.

partitioned a hd to run 98/95. 98 is great but after installing OEM version of 95 and then rebooting i get:

while initializing ios device:
windows protection mode, reboot computer

nothing in bootlog.txt shows any problem. have a whole gig in the 95 partition. it won't even let me into safe mode! what gives?!
