Was this modem working fine before?
It is quite possible for a modem to be damaged and only partially work, from things such as a computer power supply failing, or from a voltage surge or spike if your computer and phone line are not protected from that, or from installing or uninstalling a modem card in it's slot when the computer power supply is powered even when Windows is Shut Down if you have an ATX power supply.

Is the phone line source from the wall connected to the LINE port on the modem? On many modems it can't actually connect properly unless the phone line is in that port - it won't work properly if it is connected to the PHONE port.
The Modem Diagnostics in Windows do not check the external phone line connection to the modem.

Pick up a phone on the same line as the modem. Do you hear noticable line noise such as static, clicks, poping? If you do that will interfere with your modem's ability to connect to the internet properly.

You could try another phone cord between the modem and the wall. If it is a typical cheap flat cable, they tend to act like an antenna and pick up electronic and electric noise - the shorter it is the better.

Do you also have a high speed DSL or ADSL connection on the same phone line? If you do, you need to install a filter in the phone line that blocks the ADSL or DSL signal from reaching the modem.
