USR 5610b MODEM dials out. Windows reports that the connection has been made, but I cant connect to anything. ping reports "no such url"
Modem was working. I took it out and put in Linux computer. I reinstalled it in Win computer, and now have problem.
Device manager reports I am using the USR driver.

If Device Manager says the modem is working properly, and the Diagnostics in Control Panel - Modems indicate it is working properly, there is nothing wrong with the drivers, probably nothing wrong with the modem, and you are having no problems with the modem getting an IRQ it can use.
- if you also have a DSL or ADSL connection on the same phone line, you must use a DSL or ADSL blocking filter between the line and the modem.
- make sure the telephone cable from the wall is connected to the LINE port on the modem - some modems will not work properly if you have that in the PHONE port.
- make sure your Dial-up Network settings are set the way your ISP requires.
- your Intenet Options (e.g. Control Panel - Internet Options) must be set to use a Dial-up connection.
- some modems will not work properly if ther IRQ is shared with another device.
Look in System Information - Hardware Resources - IRQ's to see whether the IRQ the modem is using is shared with another device other than IRQ Holder...
Avoid using the modem in the PCI slot at the end closest to the center of the mboard - that slot is usually forced to use the same IRQ as the video card in an AGP slot, or the onboard video.
- if you installed the modem in a slot it wasn't in before, try installing it in the slot you had it in before.
- you can't have anything else using the modem when you are trying to connect to the internet, such as a fax program waiting for a call.

Not using DSL or ADSL
cable from the wall is connected to the LINE port on the modem I can hear the modem dial out. Status reports that I am connected at 28K, which is usual here,
(Cable from wall is currently plugged in the computer I am using right now, so it is good!)

Assuming the modem log is that of the computer and modem that is not connecting to any url.....
Your modem appears to be working fine, and is connecting at 28.8.I've seem some instances where you can use several versions of drivers for a modem but not all of them work properly - some things work, some things don't.
Are you certain the drivers are the ones for your exact modem model and chipset?Do you have the Dialer application installed in Windows?
(Programs - Accessories - Communications - Dialer)
If you don't you can load it in Add/Remove Programs - Windows Components - Communications.
Try the Dialer to see if it works.Do you have a Fax program? If you do, can you send a fax to yourself or to someone else?

It could be something in your existing configuration is corrupted.
You could Remove the modem in Device Manager, delete your Dial-up Networking configuration, reboot, install the drivers for the modem (make sure you have the right ones) which should also re-establish your Dial-Up Networking configuration, check the Dial-Up config to make sure it is set as to the the ISP's requitements, check Internet Options to make sure it is using the Dial-Up connection.

Since the modem seems to be working fine, the following may not help, but it's a good last resort, especially if your problem is caused by a conflict that isn't showing up when the modem is installed.
It is possible changing the IRQ the modem uses will get it working properly.
Sometimes even if nothing else has changed, removing a card or disabling a device can shift what resources are assigned to what, and if you put back the card where it was, or enable the device again, the resources may not shift back to they way they were just before you removed the card or disabled the device.
e.g. I guy who posted here not long ago experienced that by merely unplugging his PS/2 keyboard, I think with the bios set to autodetect it. When he plugged it back in the resources did not shift back and a card that worked before would not work.In your case you may have a conflict that is not showing up in Windows.
I don't like that it is sharing IRQ 5 with all those other things, especially the video - lots of things don't get along with sharing an IRQ with the video. "Nvidia nForce PCI System Management"
Is that something to do with the Nvidea mboard chipset?
However, devices built into the mboard sharing the same IRQ rarely if ever causes problems, so the only thing it would be desirable to have use a different IRQ is the modem.
If you are not using a PS/2 mouse, or if you are but can use a serial or USB connected mouse instead, disable PS/2 mouse support in your bios setup - that frees up IRQ 12 for general PNP use - modems rarely can use that IRQ, but some other devices can e.g. USB controllers, some Network cards or chips, and possibly make the IRQ they were using availble to the modem.
- if you are not using one or both Com ports, disable one or both in the bios to free up the IRQ's they use for general PNP use - e.g. modems can often use IRQ 4 or 3
- In the case of USB controllers, to get them to use a different available IRQ you may have to disable all of them in the bios, reboot, load Windows, restart, enable the controllers in the bios.
- If none of that helps shifting which IRQ is used by the modem, try the modem in another slot.
