I have set an FTP site on SBS2003 and it works great on the local network. I cannot access it from outside though. I have forwarded port 21 to the server and have seen login failures on the server after I attempted to access the site externally.
I have seen posting about allowing access to the FTP site from the external IP, but cannot find where I would enable this.
The login I used from outside is the same I have used successfully from inside. Any ideas?

FTP filter configuration is more complex at the firewall, if the firewall did not support stateful filters. FTP needs Port 21, 20 and any high port between 1024 and 65535. Stateful packet filter opens the ports as needed and closes them when the connection ends.
So check your firewall logs for any blocked communication that might be a part of the ftp connection.
