Windows Explorer Fails to Open 1st Time

When I click iexplore.exe, Explorer fails to show all paths and closes. The following error is reported:

has generated errors and will be closed by windows. You will need to restart the program. An error log is being created.

Explorer then works correctly when clicked a second time.

If anyone knows how I can correct this I would really appreciate your advise.

PLEASE NOTE:

The following line should have been placed BEFORE "has generated errors":

(process has already exited)has generated errors and will be closed by windows. You will need to restart the program. An error log is being created.
