Just solved a USB lockup problem:
Symptom - when ever I unplugged and repluugged the hub with my USB mouse and keyboard, I had to reboot in order to get them working again.
I traced the problem to my Karna Boomslang USB mouse driver - if I installed the driver on a W2k box the problem appears. Uninstalling the driver did not fix the problem, deleting driver registry keys did not fix the problem (maybe I missed a few)...luckily I made a drive image just before installing the driver. Reimaging the drive made the symptom go away.
