How do i create a little window for someone to create a username and password? All I want it to do is for it to send me their message in a message window. I want to manually enter every one for quality control. I know some HTML so please walkme through whatever their is.

That's not going to cut it. You will need to use some sort of server side scripting language (PHP/ASP/etc.) to do that properly. You will need to find out if your server host supports any such languages.
