Try www.dtp-aus.com for free cgi-forms. The owner wrote them all himself, and they all integrate together with a master control panel. You will need to sign up to get a member number, which you then use as your password to download.
You do not get spammed as a result of signing up, this simply helps him keep track of who uses his files, as he has strict rules about illegal or inappropriate sites not benefitting from his work.
