SCSI drive crashed on the kernel

Hi

My lab computer is loaded LynxOS, a version of Unix. The computer has an IDE hard disk and a SCSI hard disk. On SCSI controler, there are four 72 DAT SCSI tape drives.

/dev/sdmpta.0 is the device driver of SCSI hard drive.
I encountered the problem that I could not access it. It was crashed on the kernel with the message below

Unknown error SCSI ID:0, block 0
Bad kernel memory access: in process 0 PC: 0xb80if730 fault:0x50

What reason causes this message ?
