Removing generic 22.ekq.... how?

Specs: Windows 64
I have a trojan horse generic22.ekq on my computer and cannot remove it. I've tried AVG paid, which finds it and indicates that it has been removed, but it's still there. I've run TDSSKiller and it couldn't find it. I've cleared my browser cache and Java cache..... nothing seems to work. Anyone have any ideas how to kill this bugger?

General clean up and Prep (Do prior to any AV scans)
