Instant Access won't go away!!!

Please help me again, I've already posted this, but the solutions that were suggested didn't worked... EVERY TIME I turn on my machine, appears a shortcut on my desktop and TWO on my toolbar... I've already tried AVG, SpyBot S&D, and Ad-Ware and they didn't solved the issue...
Again, forgive me for my english, but it's because I'm not American Any Help will do.

There is two types of instant access... one is a porn dialer the other is a scanner utility !
if its the porn dialer you have goto start -- run---and type msconfig goto the startup section and look for the entry that says instant access....UNTICK it...close boxes and restart pc... when back into windows goto network connections or dial up connections and look for a dial up connection that isnt your default one..I.E if you have aol installed you'll see anther one that says xxx for example.. DELETE the connection...
then update ADAWARE and run a test... if that doesnt find the dialer try AVG if that doesnt find it you might have a scanner utility installed
