I would like to run a survey from a floppy disk and for the answers to go into a csv file that could be imported into Microsoft Excel 5.0

title,surname,firstname,telextn,
So everytime i put the disk into a new computer and run it, it will prompt me for the details above and store it in CSV file, lets call it ip.csv
I would also like the output to ipconfig /all to be displayed in the csv file

DNS Servers
are all titles and the answers all appear in the appropriate place after ipconfig is ran.
i probably haven't made this very clear, but i am about to get kicked out of the internet cafe. i will check back tomorrow.
What basically we want ot do is to run ipconfig /all and for the answers to be output to a csv file together with personal info of the person sat at the computer.
We would like the program to use ordinary basic like

What you want to do is impossible with batch files in Win9X. It's possible in VBA, but probably not without additional downloads.

VBA would probably work, but you'd need some MS Office application (probably Excel) on every machine.

I can see that maybe i made this problem too complicated.

Forget all the stuff in post one, and see if you can help me with this.

Lets suppose i need to collect some personal information in a survey
Title (Mr, Mrs. etc), First name, surname, telephone number etc.
The questions are asked by running a batch file, but the ansers are stored in a CSV file. A new record is stored for each person

So for example
Lets call the file, info.csv

Title,FirstName,Surname,TelephoneNo.,
Mr.,Fred,Bloggs,0123456
Mrs.,Freda,Bloggs,098765

This showing the results of input into info.csv after two surveys.
