Monitor log using SHELL or PERL

I need to write a shell or perl script that constantly monitors the contents of a log file for certain flags. I don't mean read the log file at select intervals -- I can do that with a script and crontab.

I know it's possible to do this, but does anyone know how and can you point me in the right direction? Does a module need to be included for a Perl script?
