How do I make custom templates

I would like to know if their is anyway to make a fill in the blank template for my pocket pc like they have in notes EXAMPLE
(blank note, meeting notes, & so on)

I have a pocket PC HP Ipaq 5555

I am also currently looking for any software that will communicate with office access
