Word headers want it fixed template

I work in security job and i have created a investigation report using Microsoft Word 2000. Simple form with headers and text field (wish i could attach it) I am interested in knowing to how to make the green highlighted headers non-deletable i.e headers form a template (i.e in one row i have four columns and row beneath that for text entry. The top row has four header hence the four columns. How to make that as template i like to know.

To save as a template:
File Save As Save As Type: Document Template (*.dot)
It will be a template, but as long as the users can write to the file, they may figure out how to delete the header/footer. You can't protect only part of a document unfortunately.
