Any word processor. All you need to do is use Manuscript format and submit the manuscript for publishing.

It depends on who publishes the book. Some publishers only do certai formattng. If you are publishing/printing it yourself, and text editor, word processor, desktop publishing program that does your formatting requirements.
Thank you very much for your advices. I want a program that can help me to "create and maintain" a "table content with page numbers" when I modified a book. Can Microsoft word 2003 or 2007 do this?

In Word - select Insert - Reference, Index and Tables and you may select from the tabs (Table of Contents and the formatting desired) Word Help will assist in bookmarking the subheading that will populate the table of content.

Just remember to use Styles in the Word document for the chapter titles. This makes it much easier to maintain your table of contents
