Are you asking for help or just telling us what you need?

You could just put a formula in that adds 45 days in one cell and in another cell put in a formula that adds 60.

i.e. if your intake date was in cell A1 and had a value of 6/8/2010, in B1 just put in =A1+45 in C1 put in =A1+60.

Assuming that is what you are asking.
