Automated cross-referencing

I have a 600 page document with all the heading fileds styled & numbered correctly, but there are tons of cross-referneces to these fields which have not been hyperlinked. The references are always indicated by the heading numbers in square brackets. Can anyone suggest how to write a macro to do the following;
- get each heading number in the document
- search for references to this heading number, usually like [10.2.3]
- replace the reference with a field code or hyperlink

It's for a word document on MS Office word 2003.
